How should I proceed?

- Write an informal request (letter) stating:
- your identification data (name, surname, birth number, address) and
- the subject of the request (subsequent inclusion of missing insurance periods).

Attach credible proof of the missing period of insurance to the application. If you don’t have such proof, please state in your application how long your insurance lasted and the address of your employer during that period.
- Send the request to:
- to the Social Insurance Agency headquarters (Sociálna poisťovňa, ústredie, Ul. 29. augusta č. 8, 813 63 Bratislava 1); or
- via your electronic mailbox if you have activated it and you use an electronic citizen’s identity card – eID (you can find more information at the Central Government Portal slovensko.sk)
- The Social Insurance Agency will verify the data you have provided for the subsequent inclusion of the insurance period, and if it assesses it as correct, it will send you a written decision on inclusion.
